• Your Player ID
• Your date of birth
• Your residential address
• Your 14-digit Postal Request Code
• This statement:
   “I am submitting this written request because I wish to receive complimentary Sweeps Coins in order to participate in the sweepstakes promotions currently being offered by Dime Sweeps Casino. I confirm that I have read, understood, and agree to the Terms and Conditions and Sweeps Rules.”

6. Sign and date the card (MM/DD/YYYY) directly under the statement.

7. Mail the envelope.
