Your residential address (from your account)

This statement:

 “I wish to receive Sweeps Coins to participate in the sweepstakes promotions offered by Gains.com. By submitting this request, I declare that I have read, understood, and agree to be bound by Gains.com's Terms and Conditions and Sweeps Rules.”

5. Mail the envelope within 10 days of generating your code.

6. Each valid request earns 3 Sweeps Coins, credited to your account after verification.

Requests must be mailed from the same state or province as your verified address.

Only you may submit your request — no third parties or services.

Do not reuse, share, or copy request codes or cards.

Late, expired, duplicate, or automated requests are void.

Unreadable or incorrect requests will not be credited.
