Use white paper, at least 3” x 5”, written in black ink and by hand.

Write on one side only, in this order:

1. “Chip’n WIN Free Entry Request”
2. Your Chip’n WIN account number
3. Your unique 12-digit Sweepstakes Coin Request Code
4. Your full name (as on your government ID)
5. Your date of birth
6. Your phone number
7. The email linked to your Chip’n WIN account
8. Your residential address

3. Get Your Request Code

Sign into your Chip’n WIN account.

Click the button to generate your unique 12-digit code.

Write it down or screenshot it — it can only be used once.

4. Mail Your Request

Place one Request Card per envelope only.

Mail it from the same state as your verified account address.

5. Important Rules

• Each valid Request Card earns 1 Sweepstakes Coin.
• Handwriting must be clear and readable. Illegible requests are void.
• Only you may write and send the request.
• Requests from other people, services, or different states are invalid.
• Shared or reused codes will be disqualified.
