1. Prepare Your Envelope

Use a stamped #10 envelope.

Handwrite your return address in the top-left corner.

Write “Sweeps Entries” on the front.

Address it to:

2. Prepare Your Request Card

Use a 4” x 6” white card or paper, unfolded, blank, and unlined.

Handwrite on one side only in this order:

• Top-left corner: Your unique, single-use Postal Request Code
• Below that:
  • Your full name (as shown on your government ID)
  • The email registered to your Clubs Poker account
  • Your state of residence (as listed in your account)
  • This statement exactly:

"I wish to receive Sweeps Coins to participate in the sweepstakes promotions offered by Clubs Poker. By submitting this request, I hereby declare that I have read, understood and agree to be bound by Clubs Poker’s Terms of Use and Sweepstakes Rules."

3. Mail Your Request

Place one Request Card per envelope only.

Mail it from the same state or province as your verified address.

4. Rewards and Limits

Each valid Request Card earns 3 Sweeps Coins.

You can send up to 50 requests per sweepstakes period.

Extra requests beyond 50 will not count or carry over.

5. Important Rules

• Handwriting must be clear and legible, or your request will be void.
• Only you may write and send the request.
• Requests from others, or mailed from a different state, are invalid.
• All entries are subject to account verification and rules compliance.
