Step 1: Get Your Mail-In Code

Before you write your card, get your Mail-In Code (a unique code that identifies your account).

To get it:

1. Log into your High 5 Casino Player Account.
2. Click the sidebar, scroll down, and select “My Account.”
3. Click “Get Mail-In Code.”
4. Your Mail-In Code will appear—write it down exactly as shown.
   
   

Notes:

• Each code can only be used once and expires after 60 days.
• You must not share your code—it’s linked to your account only.

Step 2: Write Your Request Card

Use a 4x6” postcard or blank piece of paper cut to that size (unfolded, unlined, one side only).
Use one color of ink (black ink recommended) and write clearly by hand.

On the front (landscape layout) of your Request Card, write:

1. Top-left corner:
   • Your full name (as shown on your Player Account)
   • Your email address (same as your Player Account)
   • Your Mail-In Code (exactly as displayed)
   • Your residential address (same as on your Player Account)
     
     
2. Top-right corner:
   • The date you prepared the card
     
     
3. Below that information:

4. “I wish to receive Sweeps Coins to participate in Sweeps Play on High 5 Casino.”

Step 3: Prepare and Address Your Envelope

Use a standard white #10 envelope (unfolded, unlined).
Handwrite everything neatly in one color of ink.

On the envelope front:

• Top-left corner: Your return address
• Back (centered): Your Mail-In Code

Step 4: Mailing Rules

 One (1) Request Card per envelope
 Must be mailed from the same state as your verified address
Must be handwritten (no typing or printing)
Handwriting must be legible — unclear requests are void
You must send it yourself (not through a service or third party)
No automation or copying — each request must be original

Step 5: After You Send It

• If your request meets all requirements, 3 Sweeps Coins will be added to your account.
• High 5 Casino is not responsible for lost, late, or invalid requests.
• Tampering or outsourcing handwriting will result in disqualification.
