1. Get an Envelope Request Code

• Log in to your Modo account.
• Click “Envelope Code Request” and follow the prompts.
• You’ll receive a 12-digit code — write it down carefully.
• Each code is good for one use and expires after 30 days.

2. Prepare Your Card

• Use a white, 4x6 index card, unfolded and unlined.
• On one side only, handwrite clearly in this order:
  1. Your full name (as shown on your ID)
  2. The email linked to your Modo account
  3. The residential address on your Modo account
  4. Your 12-digit Envelope Request Code
  5. This statement:
      “Sweepstakes Entry! I wish to receive Sweepstakes Coins to participate in the [Insert Month] Sweepstakes Contest offered by Modo. By submitting this request, I hereby declare that I have read, understood, and agree to be bound by Modo’s Terms of Use and Official Rules.”
  6. Sign your name and write the date (MM/DD/YYYY) directly below the statement.

3. Address Your Envelope

• Place your completed card inside a white #10 envelope.
• Handwrite the address:

• Also handwrite your return address on the envelope.

4. Mail Your Request

• Only one card per envelope.
• You must handwrite everything yourself — no printed or assisted writing.
• The signature must match the one on your Modo ID.
• Send your envelope from the same state as your registered address.

5. What You’ll Receive

• Each valid mail-in entry earns 3 Sweepstakes Coins.
• Coins will be added to your account once your request is verified.
