- Use a 4x6 inch unlined index card (no plain paper).
- Handwrite in black or blue ink only.
- On the top-right corner, write the date.
- On the top-left corner, write your Postal Request Code.
- Below that, write in order:
- Full name (as per ID & account)
- Account email
- Registered residential address
- Then write exactly:
