1. What You Need

• A 4x6 inch postcard or piece of white paper (unfolded)
• A stamped envelope (#10 size is fine)
• A pen — use blue or black ink
• Your Stake.us account must be verified

2. Address the Envelope

Handwrite on the front:

• Your return address (top left corner)
• The words “Stake Cash Credits”
• Mail it to:

3. Fill Out the Request Card

On one side of the 4x6 card, handwrite:

1. Your full name (as on your ID)
2. Your Stake.us username
3. Your registered residential address
4. Your account email address

6. This exact statement:

“I wish to receive Stake Cash to participate in the sweepstakes promotions offered by Stake Sweepstakes. By submitting this request, I hereby declare that I have read, understood, and agree to be bound by Stake’s Terms and Conditions.”

4. Important Rules

• Only one request card per envelope
• Everything must be handwritten by you — no copies, stencils, or printed text
• Writing must be clear and easy to read
• The request must be mailed from the same state as your verified address
• Each code is unique and can’t be reused or shared
